The Migrant and Seasonal Agricultural Worker (MSAW) Supervisor works in collaboration with the MSAW Clinical Lead to coordinate primary care and enabling services for migrant and seasonal farmworkers in Delaware. The MSAW Supervisor develops and maintains relationships with farm owners, growers and other community agencies servicing migratory and seasonal agricultural workers and their families located throughout Delaware. In addition, the MSAW Supervisor is the direct supervisor for the Community Health Outreach team. The MSAW Supervisor is responsible for ensuring that needs of the migrant and seasonal farmworkers and their families are being met including connecting to medical, dental and behavioral health services at Westside Family Healthcare, promoting preventive healthcare and healthy living, and connecting to community resources.

- Oversee the day-to-day logistics of the Migratory and Seasonal Agricultural Worker Program and community health outreach efforts in Delaware offered by Westside Family Healthcare, ensuring this population has access to care and enabling services either at their designated farm, housing camp or on site at WFH facility. Coordinate the logistics of care for MSAW population, both on site and connecting in the community to meet needs.
- Develop and maintain relationships with farm owners, growers, crew leaders, farm workers and other community agencies servicing migratory and seasonal agricultural workers and their families.
- Supervise and provide ongoing development of MSAW outreach team, including maintaining team schedule to ensure adequate coverage, completing performance reviews, orientation and training; performing routine quality audits to ensure adherence to program requirements and achievement of grant deliverables. Participate in the recruitment, interviewing and orientation of new MSAW team members. Provide ongoing training, support, additional training and remediation as directed by Director of Clinical Operations or MSAW Clinical Lead; and continuously practice, enforce and educate staff regarding safety standards.
- Ensure that program activities are conducted in accordance with grant requirements and deliverables, as well as within the scope of WFH policies and procedures. This includes keeping a database of contacts with notes, i.e. community partners, farms, community resources, MSAW non-clinical inventory, etc.
- Serve as a health advocate by offering information to individuals in the community regarding the importance of preventive primary care, what this means, and how Westside Family Healthcare meets the needs of the communities through the patient-centered medical home (PCMH) service delivery model.
- Establish and maintain relationships with other agencies and organizations in the community to meet community needs, plan outreach events and to ensure that services are not duplicated. Identify community organizations or events where outreach activities can be conducted. Collaborate with Westside Family Healthcare leadership to coordinate WFH's participation in activities to connect migrant and seasonal agricultural workers with clinical services and to promote services offered by Westside Family Healthcare.
- Collaborate with the MSAW Clinical Lead to coordinate care for clinical services, and communicate all last-minute schedule changes to all affected personnel.
- Attend community meetings and participate in community events/fairs to support the care for migrant and seasonal agricultural workers.
- Maintain compliance with all professional and organization requirements to meet the positions standards
Minimum Objective Qualifications:
- Five years of experience in the healthcare field, including experience working with vulnerable populations.
- Supervisory experience
- Ability to work nights and weekends
- Six months of experience using Microsoft Office software suite and minimum six months of experience with electronic medical record software
- Ability to travel independently to locations (reliable transportation, insured vehicle and valid driver's license)
Preferred Objective Qualifications:
- Bachelor's degree, preferably in social work or a related social science of human services field.
- Experience using Microsoft Word, Excel and Outlook
- Experience, training and/or passion to provide quality care to underserved populations
